Most families only hear what their loved one chooses to share. Our coordinator calls your loved one every week — on a day with no caregiver visit — and asks the questions families are afraid to ask. How did you sleep? Is anything hurting? Are you eating? How is your spirit today?

What the coordinator hears shapes the care plan. Patterns are tracked week over week and reviewed by your RN at every 60-day clinical review. A mood shift that appears once is noted. One that appears three weeks in a row becomes an action item.
