What the assessment is
It’s a comprehensive, in-home clinical evaluation performed by one of our RNs — in Maryland, a structured 13-section review conducted in the home with your family present. It usually takes 60 to 90 minutes, and it is free with no obligation to proceed.

The RN reviews health conditions, medications, mobility and fall risk, cognition, nutrition, home safety, and daily routines to build a plan matched to real needs.
The nurse looks at the whole picture: current diagnoses and medications, mobility and fall risk, cognition and mood, nutrition and hydration, home-safety hazards, and — just as important — the daily routines and preferences that make your loved one themselves. This is where CMT-certified medication needs and dementia-specific risks are identified.

You leave the assessment with a written, personalized care plan — the specific tasks, hours, and clinical oversight your loved one needs — and clarity on cost.
